Enter Name, Phone Number, E-mail address, and who your money will be paid  to at the top of the form. Please verify that your e-mail is correct, as this will build the distribution list for standing updates. If you are submitting multiple entries, please put a number after each entry, including the first one, using the "#1, #2" format.

Select teams you want to win by clicking on them, starting with Round 1.  Once you have selected a team as a winner, they will be made available for selection for the next round.  Please note that if you change a pick, make sure all games affected are changed to reflect the new team.

Enter tie-breaker information for how many TOTAL points will be scored in the championship game. (Note that this is the third tie-breaker...the first is Correctly selecting winner, and second is umber of teams in the final four.)

Once your bracket is complete, click the 'submit' button at the bottom of the page.  The form will be checked to make sure all games have been filled in.  Upon verification, your pick sheet will be available for immediate printing.  Please print for your records.

Return to the entry sheet screen to enter additional pick sheets.

You're done!  Sit back and wait for the madness to begin!
